What to Expect at your Appointment

We appreciate the trust and confidence you place in us to provide you with your best eye health and sight correction.

On the day of your scheduled office visit:

  • Please complete and submit all forms under the patient forms.
  • Bring any eye health or medical insurance cards.
  • Bring a list of all medications you are currently taking.
  • Bring your current eyeglasses, current sunglasses and reading or computer glasses with you.
  • AND the means to pay any deductible, co-pay, or overages the day of your scheduled visit.

Once you have scheduled and reserved an appointment in the unfortunate event you are unable to keep your appointment, for any reason, we kindly request your 48 hour advance call to reschedule.

Fees

We request payment for your non-covered eye health fees at the time services are provided, including your responsibility for co-payments and deductibles.

What to Expect

Here’s what you may expect and what we request in order to make your office visit effective and efficient for you.

It is a 6 step process and we will be with you for approximately 1.5 hours.

  1. Initial Testing
    A technician will complete tests that the doctor needs before the doctor examines your eyes.
  2. Doctor Evaluation
    The doctor will examine the health of your eyes utilizing the Clarifye system review the findings, address your questions, and provide advice and choice of vision correction if needed.
  3. Spectacle selection and/or contact lens evaluation:
    A LensCrafters associate can assist you with selecting eyewear if you would like.The eye doctor will discuss the appropriate contact lens modality with you, evaluate your eyesight with them and proper centration and fit of the lens.
  4. Dilating Your Eyes
    Often, dilation is necessary for the doctor to examine and evaluate your eye health. The dilation step requires about 25 minutes for the pupils to become large enough for the eye doctor to see inside your eye. This is painless and important to ensure the best eye health
  5. Pre-appoint and Checkout
    We will schedule your yearly exam at checkout and collect any fees or co-payments from you on the day of service.
  6. Your Scheduled Return
    In the event you will return for additional diagnostic testing, at the request of the doctor, we will schedule for your convenience.

To DECREASE your wait time at the office BEFORE YOU ARRIVE, fill out all forms, and BRING all finished paperwork to your appointment 15 minutes before your reserved doctor visit.

Dr. Rad Patel 1989-2023

It’s with a heavy heart that we share the passing of Northlake Eyecare Associates, Dr. Rad Patel at the young age of 33. His shining personality and humor brought happiness to everyone he encountered.

Dr. Patel graduated from the Inter-American University of Puerto Rico School of Optometry in 2018. He completed an externship at OMNI Eyecare Services and was chosen as a resident in Ocular Disease. He completed his residency with a diploma from UAB in 2019. He then joined the established optometry practice of Dr. Steven R. Warstadt & Associates in Atlanta. In 2021, Dr. Patel became the sole owner, and the practice became known as Northlake Eyecare Associates.

With regret, Dr. Patel passed away on Friday, May 12, 2023. He was taken too soon, and we will miss him deeply. Northlake Eyecare Associates is still operating for all patients.
